Oral Health Literacy Instrument Malay Version (OHLI-M)

Introduction: Oral health literacy refers to the degree to which individuals have the capacity to obtain, process and understand basic health information and services needed to make appropriate oral health decisions. Oral Health Literacy Instrument (OHLI) is measuring the functional oral health literacy in terms of ability to comprehend oral health messages and numbers on medicine labels prescribed in dentistry, dental appointment card, and post-operative instructions.

Objectives: To translate Oral Health Literacy Instrument to Malay language (OHLI-M). Subsequently, this study aimed to determine its validity (face, content, construct, and criterion) and reliability (internal consistency and test-retest reliability) of OHLI-M.

Materials and Methods: Cross-cultural adaptation of OHLI was conducted in Phase I.  The original OHLI was translated into Malay language (OHLI-M) by using forward and backward technique with committee approach. Phase II was cross-sectional study to test psychometric properties of OHLI-M.  One hundred and ninety-five Malaysian adult respondents were selected via systematic random sampling and requested to fill up (OHLI-M) and a short Malay version of Test of Functional Health Literacy in Adults (S-TOFHLA-M). Eighty seven out of 195 respondents were selected for clinical examination to investigate caries experience using the index of decay, missing, filled teeth (DMFT) and periodontal status using Community Periodontal Index (CPI). Fifty out of 195 respondents were selected through convenient sampling to participate in test-retest of OHLI-M after 2 weeks from first administration. Data analysis was performed by using IBM SPSS 22.0. Construct validity of OHLI-M was assessed by determination of convergent validity and known group validity. The convergent validity was determined by correlating Reading Comprehension section of OHLI-M score with S-TOFHLA-M score using Spearman’s rank correlation. The known group validity was tested by comparing OHLI-M score with the different levels of education and timing of last dental visit using One-way ANOVA.  Concurrent validity was assessed by correlating between OHLI-M with DMFT and CPI using Pearson’s correlation. Internal consistency and test retest reliability were assessed by Cronbach’s alpha and intra-class correlation coefficient respectively. The significant level was set at less than 0.05.

Results: Face and content validity of OHLI-M were satisfactory. Mean (Standard Diviation) age of respondents was 32 (SD = 10.5) years and 56.4% of them were female. Majority 78.5% were Malay and 44.6 % of respondents were educated until SPM level and below.  In last two years, 62% had visited a dentist. Total mean score of OHLI and S-TOFHLA-M were 75.1 (SD=15.64) and 94.0 (SD=11.81) respectively. Respondents with low education had significantly lower oral health literacy than middle and high education    (p-value=0.02). In Reading Comprehension section, OHLI-M score was correlated with S-TOFHLA-M score (Spearman`s rank correlation coefficient = 0.37). No associations were found between OHLI-M with DMFT and CPI. The Cronbach’s alpha values of OHLI-M and its components were high (more than 0.7). The ICC values indicated a very good agreement between the test and retest results for OHLI-M (more than 0.8).

Conclusion: This study has established the satisfactory equivalence between OHLI-M and original OHLI. The OHLI-M is valid and reliable instrument to evaluate functional oral health literacy among adult in Malaysia.
